For plans effective January 1, 2019, members enrolling in Keystone 65 Preferred HMO or the Choice Plus program should select the Concordia Choice Plus network. Members enrolling in the Choice program are required to select a primary dental office and should select the Keystone DHMO Network.

For plans effective January 1, 2020, members enrolling in Keystone 65 Preferred HMO and Keystone 65 HMO and Personal Choice 65 PPO and Prime PPO members enrolling in the Choice and the Choice Plus programs should select the Concordia Choice Plus network.

Group plan Keystone 65 HMO members with dental benefits are required to select a primary dental office and should select the Keystone DHMO network.
