Introducing Strive Health

We understand it can be stressful and complicated to manage your care for certain conditions, including end-stage renal disease (ESRD) and chronic kidney disease (CKD). That’s why Independence Blue Cross (Independence) is working with Strive Health (Strive) to offer you a program that provides assistance with coordinating your care.

By participating in the Strive Health Program, you’ll be supported by Kidney HeroesTM, a compassionate team of nurse practitioners, care managers, and other health care team members who specialize in kidney disease. Your Kidney Heroes team will also work closely with your primary care provider (PCP) and nephrologist (kidney care specialist) to keep them informed of your condition and the services provided to you.

How Strive Health can help

Your Kidney Heroes team will help guide you so that you can make the best decisions for your health. As a member of the Strive Health Program, you will:

  • Work with your Kidney Heroes to understand kidney disease, what you can expect as a member of the program, and how you can best manage your health to achieve your treatment goals
  • Have access to a range of educational materials, as well as informative mobile and/or face-to-face sessions at every step of your health care journey
  • Have more quality time with a health care professional to discuss your ongoing health care needs and answer any questions you may have
  • Be able to connect with the right specialists to address any health care needs
  • Better understand when and how to take your medications, receive healthy eating tips, and get general health advice to help you stay active and feeling your best

How to enroll
